Access the Patient Portal

VCHS offers the VCHS Medical Clinic Patient Portal, Follow My Health, as a way for you to view information related to your recent clinic visit.

Instructions for Patient Portal:
1. Click on the Follow My Health Link above.
2. Select "I need to sign up"
3. Fill in the required fields (ex: e-mail, name, DOB, zip code) 
4. Create a Password
5. Select the hospital (Valley County Health System or VCHS) 

Once all steps above are complete, it will take 1 to 3 business days for your medical information to update on your account. 
If you have any issues or concerns regarding the Patient Portal, please call 308-728-4200.

The following information is available to you 96 hours after your appointment is complete:

• Major problems/procedures
• Allergies
• Vital Signs
• Procedures
• Medication List
• Lab Results
• Plan of Care

(Please note your lab results will be available once your provider has reviewed and signed off on the report.)
