Admissions

Because your time is valuable we want to ensure your admission to Valley County Health System is comprehensive and prompt. Any information provided before your admission helps in the overall process. Many of these items we request you bring prior to admission are required by government agencies. Whether you will be an inpatient (staying overnight) or an outpatient (going home the same day as services are provided) you will need to complete the admission process and provide appropriate documentation.

Below is a list of required items for admission to the hospital:

• Photo ID (e.g. drivers license)

• Names of healthcare provider (the name of the medical provider requesting services and your personal healthcare provider if different from the one requesting services)

• Current insurance card(s)

• Medicare and/or Medicaid cards

       Medicaid Share of Cost form, if on Medicaid

• Advance Directives (one or both of the following)

       Power of Attorney for Health Care

       Living Will

• Guarantor (person responsible for payment of services) information

       Date of birth

       Social Security number

       Employer (name, address, phone numbers)

       Emergency contacts (name, address, phone numbers)

Please report to the front desk of the hospital for all registrations except for emergencies.

Upon arrival you will be seated in a registration booth, staff will verify your appointment, review your demographics, advise you of the Privacy Policy and acquire a written consent to treat you.

It is the policy of Valley County Health System to utilize armbands to identify patients who are present for hospital services.  An armband must be worn by all patients who have been admitted for services, including emergency room, inpatient, swing bed and all outpatient services.

Registration office hours are Monday through Friday 7 a.m. to 5:00 p.m. and Saturday from 8 a.m. to 12 p.m.  Admissions after hours are done by acute care staff. If you have any questions, or for more information please call 308.728.4200.
